FELLOWSHIP REQUEST FORM

If your committee/organization desires help of the Fellowship Committee for a paticular event, please submit this form to the Fellowship Committee. See instructions below.

  1. Fill out the form online or print a copy and fill it out by hand.
  2. Submit the completed form to the Fellowship Committee well in advance of the event and no later than the first Thursday of the month--Fellowship's current meeting time.
  3. You may give the form to a committee member or leave it in Fellowship's box in the church office.
